Cystic fibrosis Uncertain:3
The p.G1343S variant (also known as c.4027G>A), located in coding exon 25 of the CFTR gene, results from a G to A substitution at nucleotide position 4027. The glycine at codon 1343 is replaced by serine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
This sequence change replaces glycine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 1343 of the CFTR protein (p.Gly1343Ser). This variant is present in population databases (rs747324955, gnomAD 0.009%). This variant has not been reported in the literature in individuals affected with CFTR-related conditions. ClinVar contains an entry for this variant (Variation ID: 495939). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt CFTR protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

CFTR-related disorder Uncertain:2
The CFTR c.4027G>A variant is predicted to result in the amino acid substitution p.Gly1343Ser. To our knowledge, this variant has not been reported in the literature. This variant is reported in 0.0097% of alleles in individuals of European (Non-Finnish) descent in gnomAD. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -

Variant summary: CFTR c.4027G>A (p.Gly1343Ser) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4.4e-05 in 251174 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in CFTR causing Cystic Fibrosis (4.4e-05 vs 0.013), allowing no conclusion about variant significance. To our knowledge, no occurrence of c.4027G>A in individuals affected with Cystic Fibrosis and no experimental evidence demonstrating its impact on protein function have been reported. ClinVar contains an entry for this variant (Variation ID: 495939). Based on the evidence outlined above, the variant was classified as uncertain significance. -
