This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. Mutations in this gene are associated with ataxia telangiectasia, an autosomal recessive disorder. [provided by RefSeq, Aug 2010]

The tyrosine at codon 2817 is replaced by cysteine, an amino acid with highly dissimilar properties. This alteration has been identified in multiple individuals diagnosed with breast cancer (Lin PH et al. Oncotarget. 2016 Feb;7:8310-20; Kwong A et al. J Mol Diagn, 2020 Apr;22:544-554). This variant was also observed in 1/3251 individuals who met eligibility criteria for hereditary breast and ovarian cancer syndrome (Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |

This frequency is not significantly higher than estimated for a pathogenic variant in ATM causing Breast Cancer (4e-05 vs 0.001), allowing no conclusion about variant significance. c.8450A>G has been reported in the literature as a VUS in settings of multigene panel testing among individuals with early-onset/familial breast cancer or in a cohort of high-risk breast cancer patients who tested negative for BRCA1, BRCA2, TP53 and PTEN genes (example, Han Lin_2016, Kwong_2020). These report(s) do not provide unequivocal conclusions about association of the variant with Breast Cancer/Ataxia-Telangiectasia/ATM-related cancers. The variant was not identified in LOVD 3.0. The variant was identified in control databases in 11 of 277002 chromosomes at a frequency of 0.00004 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: European (Non-Finnish) in 1 of 126610 chromosomes (freq: 0.000008), East Asian in 10 of 18858 chromosomes (freq: 0.00053), while the variant was not observed in the African, Other, Latino, Ashkenazi Jewish, European (Finnish), and SouthAsian populations. The p.Tyr2817 residue is conserved in mammals but not in more distantly related organisms, and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) suggest that the variant may impact the protein; however, this information is not predictive enough to assume pathogenicity. One study reported this variant as a "variant of uncertain significance strongly suspected of being a deleterious mutation" but this was only based on protein structure, and not confirmed with any experimental evidence (Lin_2016_26824983). The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time.
