Variant summary
Our verdict is Uncertain significance. The variant received 5 ACMG points: 9P and 4B. PM1PM2PP2PP3_StrongBS2
The NM_001370259.2(MEN1):c.485T>C(p.Val162Ala) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000342 in 1,461,670 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. V162I) has been classified as Uncertain significance.

Multiple endocrine neoplasia, type 1 Uncertain:2

This sequence change replaces valine, which is neutral and non-polar, with alanine, which is neutral and non-polar, at codon 162 of the MEN1 protein (p.Val162Ala). This variant is present in population databases (rs748648909, gnomAD 0.007%). This missense change has been observed in individual(s) with clinical features of multiple endocrine neoplasia type 1 (PMID: 33125695, 34326862). This variant is also known as p.Val167Ala. ClinVar contains an entry for this variant (Variation ID: 527277). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt MEN1 protein function with a positive predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.V162A variant (also known as c.485T>C), located in coding exon 2 of the MEN1 gene, results from a T to C substitution at nucleotide position 485. The valine at codon 162 is replaced by alanine, an amino acid with similar properties. This variant has been observed in an individual with clinical features of multiple endocrine neoplasia type 1 (Godlewska M et al. Endokrynol Pol, 2020 Oct;71:579-580). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
