Variant summary
Our verdict is Likely benign. Variant got -2 ACMG points: 0P and 2B. BP4_Moderate
The NM_024675.4(PALB2):c.470C>T(p.Ser157Leu) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000062 in 1,613,882 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★).

Hereditary cancer-predisposing syndrome Uncertain:3
This missense variant replaces serine with leucine at codon 157 of the PALB2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been detected in at least three individuals affected with breast cancer (PMID: 33471991; Leiden Open Variation Database DB-ID PALB2_011177; Color internal data). This variant has been identified in 5/282784 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.S157L variant (also known as c.470C>T), located in coding exon 4 of the PALB2 gene, results from a C to T substitution at nucleotide position 470. The serine at codon 157 is replaced by leucine, an amino acid with dissimilar properties. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

not provided Uncertain:1
This variant is denoted PALB2 c.470C>T at the cDNA level, p.Ser157Leu (S157L) at the protein level, and results in the change of a Serine to a Leucine (TCA>TTA). This variant has not, to our knowledge, been published in the literature as a pathogenic or benign germline variant. PALB2 Ser157Leu was not observed at a significant allele frequency in large population cohorts (Lek 2016). PALB2 Ser157Leu is located in the region of interaction with BRCA1 and RAD51 (Sy 2009, Buisson 2010). In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect. Based on currently available evidence, it is unclear whether PALB2 Ser157Leu is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. -
Familial cancer of breast Uncertain:1
This sequence change replaces serine, which is neutral and polar, with leucine, which is neutral and non-polar, at codon 157 of the PALB2 protein (p.Ser157Leu). This variant is present in population databases (rs749078410, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with PALB2-related conditions. ClinVar contains an entry for this variant (Variation ID: 410140). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
