Variant summary
Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_004646.4(NPHS1):c.802C>T(p.Arg268*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000167 in 1,613,176 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Variant results in nonsense mediated mRNA decay.

Variant summary: The NPHS1 c.802C>T (p.Arg268X) variant results in a premature termination codon, predicted to cause a truncated or absent NPHS1 protein due to nonsense mediated decay, which are commonly known mechanisms for disease. This variant was found in 4/118618 control chromosomes at a frequency of 0.0000337, which does not exceed the estimated maximal expected allele frequency of a pathogenic NPHS1 variant (0.0033541). This variant has been reported in at least 3 NS patients both in compound heterozygosity and homozygously. Taken together, the variant was classified as Pathogenic. -

This sequence change creates a premature translational stop signal (p.Arg268*) in the NPHS1 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in NPHS1 are known to be pathogenic (PMID: 11317351, 11854170, 12039988). This variant is present in population databases (rs749341977, gnomAD 0.008%). This premature translational stop signal has been observed in individual(s) with nephrotic syndrome (PMID: 19423745, 28204945). ClinVar contains an entry for this variant (Variation ID: 496273). For these reasons, this variant has been classified as Pathogenic. -
Not observed at significant frequency in large population cohorts (gnomAD); Nonsense variant predicted to result in protein truncation or nonsense mediated decay in a gene for which loss of function is a known mechanism of disease; This variant is associated with the following publications: (PMID: 25525159, 31589614, 19423745, 20507940, 28204945, 30594156, 33980730, 34859019) -
