A homozygous missense variant in exon 1 of the SMPD1 gene that results in the amino acid substitution of Proline for Leucine at codon 105 was detected. The observed variant c.314T>C (p.Leu105Pro) has not been reported in the 1000 genomes databases and has a MAF of 0.0004% in the gnomAD database. This variant has previously been reported in patients affected with MPS I (PMID: 31941852). The in-silico prediction of the variant is deleterious by MutationTaster, FATHMM and DANN. In summary, the variant meets our criteria to be classified as pathogenic. -

Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 249832 control chromosomes. c.314T>C has been reported in the literature in multiple homozygous and compound heterozygous individuals affected with Niemann-Pick Disease (e.g., Hollack_2012, Ranganath_2016, Cheema_2020, Deshpande_2021). These data indicate that the variant is very likely to be associated with disease. Multiple publications report experimental evidence evaluating an impact on protein function; the most pronounced variant effect results in approximately 2-3% of normal activity (Dardis_2005, Deshpande_2021). The following publications have been ascertained in the context of this evaluation (PMID: 33083013, 16010684, 34273913, 22818240, 27338287).
