Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 553752) and has been interpreted as likely pathogenic by Counsyl and pathogenic by Invitae. Of the 5 affected individuals, 1 was a compound heterozygote that carried a likely pathogenic variant with unknown phase, and 3 were compound heterozygotes that carried a pathogenic/likely pathogenic variant in trans, which increases the likelihood that the p.Arg526Cys variant is pathogenic (Variation ID: 370909; PMID: 25584046, 32170320, 27908292, 23275527). In vitro functional studies provide some evidence that the p.Arg526Cys variant may slightly impact protein function (PMID: 25584046). However, these types of assays may not accurately represent biological function. Computational prediction tools and conservation analyses suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive hyperinsulinemic hypoglycemia. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 251138 control chromosomes. c.1576C>T has been reported in the literature as biallelic homozygous or compound heterozygous genotypes in multiple individuals affected with Congenital Hyperinsulinism (example, Arya_2014, Kapoor_2013, Salomon-Estebenez_2016, Mannisto_2020, Sogno-Valin_2013). These data indicate that the variant is very likely to be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in reduced current flow under basal conditions as compared to wild-type and reduced responsiveness to diazoxide (Arya_2014). Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation.
