Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt NPHS1 protein function with a positive predictive value of 80%. This variant disrupts the p.Arg299 amino acid residue in NPHS1. Other variant(s) that disrupt this residue have been observed in individuals with NPHS1-related conditions (PMID: 20507940), which suggests that this may be a clinically significant amino acid residue. The variant allele was found at a frequency of 8.2e-06 in 245128 control chromosomes (gnomAD). c.895C>T has been reported in the literature in at least one homozygous individual affected with Nephrotic Syndrome (Schoeb_2010). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two ClinVar submitters have assessed the variant since 2014: both classified the variant as uncertain significance. The p.Arg299Cys variant is reported at a frequency of 0.00002 in the Total population of the Exome Aggregation Consortium but this is based on two alleles, so the variant is presumed to be rare. The evidence for this variant is limited. The p.Arg299Cys variant is therefore classified as a variant of unknown significance but suspicious for pathogenicity for congenital nephrotic syndrome. This variant was observed by ICSL as part of a predisposition screen in an ostensibly healthy population. - |
