Variant summary: FANCA c.3913C>T (p.Leu1305Phe) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.2e-05 in 251474 control chromosomes (gnomAD). c.3913C>T has been reported in the literature as a biallelic genotype in individuals affected with Fanconi Anemia (e.g. Pinto_2009, Castella_2011, Bogliolo_2020). These data indicate that the variant is likely to be associated with disease. Experimental evidence evaluating the variants ability to rescue FANCD2 monoubiquitination and resistance to Mitomycin-C showed the variant had similar activity to a known null variant/negative control (Bogliolo_2020). Six ClinVar submitters have assessed the variant since 2014: three classified the variant as likely pathogenic and three as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -

This sequence change replaces leucine, which is neutral and non-polar, with phenylalanine, which is neutral and non-polar, at codon 1305 of the FANCA protein (p.Leu1305Phe). This variant is present in population databases (rs753700179, gnomAD 0.007%). This missense change has been observed in individual(s) with Fanconi anemia (PMID: 19278965, 21273304, 29098742, 29904161, 31586946). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 556224). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt FANCA protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects FANCA function (PMID: 31586946). For these reasons, this variant has been classified as Pathogenic. -
