This sequence change replaces cysteine with phenylalanine at codon 1355 of the CFTR protein (p.Cys1355Phe). The cysteine residue is highly conserved and there is a large physicochemical difference between cysteine and phenylalanine. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the ExAC database. This missense change has been observed in individual(s) with clinical features of CFTR-related conditions (PMID: 28469871). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects CFTR function (PMID: 28469871). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 251244 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.4064G>T has been reported in the literature in an individual suspected with Cystic Fibrosis (Hinzpeter_2017). These report(s) do not provide unequivocal conclusions about association of the variant with Cystic Fibrosis. At least one publication reports experimental evidence suggesting an impact on protein function (Hinzpeter_2017). The following publication have been ascertained in the context of this evaluation (PMID: 28469871). ClinVar contains an entry for this variant (Variation ID: 558463).
