ACADM (HGNC:89): (acyl-CoA dehydrogenase medium chain) This gene encodes the medium-chain specific (C4 to C12 straight chain) acyl-Coenzyme A dehydrogenase. The homotetramer enzyme catalyzes the initial step of the mitochondrial fatty acid beta-oxidation pathway. Defects in this gene cause medium-chain acyl-CoA dehydrogenase deficiency, a disease characterized by hepatic dysfunction, fasting hypoglycemia, and encephalopathy, which can result in infantile death. Several computational tools predict a significant impact on normal splicing: Four predict the variant abolishes a 3 acceptor site. Consistent with this, at least one publication reports experimental evidence that this variant leads to complete skipping of ACADM exon 11. c.946-2A>C (also described as IVS10-2A>C) has been reported in the literature in individuals (compound heterozygous) during newborn screening (Catarzi_ACADM_SciWorldJ_2013, Waddell_ACADM_MGM_2006) as well as in a heterozygous carrier with reduced MCAD activity (Tucci_2021). These data indicate that the variant may be associated with disease. Three ClinVar submitters (evaluation after 2014) cite the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
