CFTR (HGNC:1884): (CF transmembrane conductance regulator) This gene encodes a member of the ATP-binding cassette (ABC) transporter superfamily. The encoded protein functions as a chloride channel, making it unique among members of this protein family, and controls ion and water secretion and absorption in epithelial tissues. Channel activation is mediated by cycles of regulatory domain phosphorylation, ATP-binding by the nucleotide-binding domains, and ATP hydrolysis. Mutations in this gene cause cystic fibrosis, the most common lethal genetic disorder in populations of Northern European descent. The most frequently occurring mutation in cystic fibrosis, DeltaF508, results in impaired folding and trafficking of the encoded protein. CFTR c.76A>G was identified in a single unpublished patient who is reported to have congenital bilateral absence of the vas deferens. It is classified as a variant of uncertain clinical significance in ClinVar by a single submitter. This variant (rs759726535) is rare (<0.1%) in a large population dataset (gnomAD: 4/250942 total alleles; 0.0016%; no homozygotes). Of three bioinformatics tools queried, one predicts that the substitution would probably be damaging, while two predict that it would be tolerated. The lysine residue at this position is conserved in all species assessed. We consider the clinical significance of c.76A>G to be uncertain at this time. -

This sequence change replaces lysine with glutamic acid at codon 26 of the CFTR protein (p.Lys26Glu). The lysine residue is moderately conserved and there is a small physicochemical difference between lysine and glutamic acid. This variant is present in population databases (rs759726535, ExAC 0.003%). This variant has not been reported in the literature in individuals affected with CFTR-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). The p.K26E variant (also known as c.76A>G), located in coding exon 2 of the CFTR gene, results from an A to G substitution at nucleotide position 76. The lysine at codon 26 is replaced by glutamic acid, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: CFTR c.76A>G (p.Lys26Glu) results in a conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 250942 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.76A>G in individuals affected with Cystic Fibrosis and no experimental evidence demonstrating its impact on protein function have been reported in the literature. Two other clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. Both laboratories cited the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
