Genes affected
KCNJ10 (HGNC:6256): (potassium inwardly rectifying channel subfamily J member 10) This gene encodes a member of the inward rectifier-type potassium channel family, characterized by having a greater tendency to allow potassium to flow into, rather than out of, a cell. The encoded protein may form a heterodimer with another potassium channel protein and may be responsible for the potassium buffering action of glial cells in the brain. Mutations in this gene have been associated with seizure susceptibility of common idiopathic generalized epilepsy syndromes. [provided by RefSeq, Jul 2008]

Uncertain significance, criteria provided, single submitterclinical testingGeneDxJan 16, 2015p.Ile60Thr (ATT>ACT): c.179 T>C in exon 2 of the KCNJ10 gene (NM_002241.4). A variant of unknown significance has been identified in the KCNJ10 gene. The I60T variant has not been published as a mutation, nor has it been reported as a benign polymorphism to our knowledge. It was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The I60T variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved through mammals. In silico analysis predicts this variant is probably damaging to the protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic mutation or a rare benign variant. The variant is found in CHILD-EPI,EPILEPSY panel(s). -

EAST syndrome Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingInvitaeSep 22, 2023This sequence change replaces isoleucine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 60 of the KCNJ10 protein (p.Ile60Thr). This variant is present in population databases (rs759993423, gnomAD 0.01%). This missense change has been observed in individual(s) with clinical features of KCNJ10-related conditions (PMID: 29191078). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 205826). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt KCNJ10 protein function. For these reasons, this variant has been classified as Pathogenic. -
Inborn genetic diseases Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsJul 24, 2016The p.I60T variant (also known as c.179T>C), located in coding exon 1 of the KCNJ10 gene, results from a T to C substitution at nucleotide position 179. The isoleucine at codon 60 is replaced by threonine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
