This variant is denoted NF1 c.6727A>G at the cDNA level, p.Ile2243Val (I2243V) at the protein level, and results in the change of an Isoleucine to a Valine (ATA>GTA). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. NF1 Ile2243Val was not observed in large population cohorts (NHLBI Exome Sequencing Project, The 1000 Genomes Consortium 2015, Lek 2016). Since Isoleucine and Valine share similar properties, this is considered a conservative amino acid substitution. NF1 Ile2243Val occurs at a position that is conserved across species and is not located in a known functional domain. In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available evidence, it is unclear whether NF1 Ile2243Val is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. -

The c.6727A>G variant (also known as p.I2243V), located in coding exon 44 of the NF1 gene, results from an A to G substitution at nucleotide position 6727. The isoleucine at codon 2243 is replaced by valine, an amino acid with highly similar properties. RNA studies have demonstrated that this alteration results in a transcript predicted to lead to a protein with an in-frame deletion of 10 amino acids; however, the exact functional impact of the deleted amino acids is unknown at this time (Ambry internal data). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site and will result in the creation or strengthening of a novel splice donor site. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
