The H22R variant in the CBS gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The H22R variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The H22R variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. This substitution occurs at a position that is not conserved. In silico analysis predicts this variant likely does not alter the protein structure/function. We interpret H22R as a variant of uncertain significance. -

The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.65A>G has been reported in the literature on one allele in a cohort of individuals with pediatric or perinatal stroke (e.g. Grossi_2020). These report(s) do not provide unequivocal conclusions about association of the variant with Homocystinuria. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 32818659). ClinVar contains an entry for this variant (Variation ID: 212870).
