Although this variant has been observed in germline cases, to our knowledge, this variant has not been reported in individuals with DICER1-related tumor predisposition (PS4 not met; PMIDs: 33718253, Internal lab contributors). The highest population minor allele frequency in gnomAD v4.1.0 (non-cancer) is 0.00001 (1/91082 alleles) in South Asian population (PM2_Supporting, BS1, and BA1 are not met). While in silico tools do not predict damaging impact of the variant on protein function (REVEL: 0.398), the splice site predictors MaxEntScan and SpliceAI indicate that the variant impacts splicing, evidence that correlates with impact to DICER1 function (PP3). This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 635 of the DICER1 protein (p.Asn635Ser). This variant is present in population databases (rs765551529, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with DICER1-related conditions. ClinVar contains an entry for this variant (Variation ID: 479636). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt DICER1 protein function with a positive predictive value of 80%. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (Invitae). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Hereditary cancer-predisposing syndrome Uncertain:2
The p.N635S variant (also known as c.1904A>G), located in coding exon 10 of the DICER1 gene, results from an A to G substitution at nucleotide position 1904. The asparagine at codon 635 is replaced by serine, an amino acid with highly similar properties. This alteration has been reported in the germline of an individual with reported papillary thyroid cancer from The Cancer Genome Atlas (TCGA) research network (Canberk S et al. Eur Thyroid J 2021 Feb;9(6):296-303). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
