not provided Pathogenic:2
Frameshift variant predicted to result in a null allele, which is not likely associated with an increased cancer risk, in contrast to missense variants that negatively impact POLE proofreading while maintaining polymerase enzyme activity, which are associated with an increased risk for colon cancer and polyps (Palles 2013, Spier 2015); Not observed at a significant frequency in large population cohorts (Lek 2016); Has not been previously published as pathogenic or benign to our knowledge -
This sequence change creates a premature translational stop signal (p.Pro1226Argfs*128) in the POLE gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in POLE are known to be pathogenic (PMID: 23230001, 25948378, 30503519). This variant is present in population databases (rs765645032, gnomAD 0.009%). This variant has not been reported in the literature in individuals affected with POLE-related conditions. ClinVar contains an entry for this variant (Variation ID: 540691). For these reasons, this variant has been classified as Pathogenic. -
Hereditary cancer-predisposing syndrome Uncertain:1
The c.3677_3678delCT variant, located in coding exon 30 of the POLE gene, results from a deletion of two nucleotides at nucleotide positions 3677 to 3678, causing a translational frameshift with a predicted alternate stop codon (p.P1226Rfs*128). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. However, loss of function via haploinsufficiency in POLE has not been clearly established as a mechanism of disease. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
