This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 1508 of the POLE protein (p.Arg1508Cys). This variant is present in population databases (rs766511597, gnomAD 0.009%). This variant has not been reported in the literature in individuals affected with POLE-related conditions. ClinVar contains an entry for this variant (Variation ID: 473671). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt POLE protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The p.R1508C variant (also known as c.4522C>T), located in coding exon 35 of the POLE gene, results from a C to T substitution at nucleotide position 4522. The arginine at codon 1508 is replaced by cysteine, an amino acid with highly dissimilar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.

DNA sequence analysis of the POLE gene demonstrated a sequence change, c.4522C>T, in exon 35 that results in an amino acid change, p.Arg1508Cys. This sequence change does not appear to have been previously described in patients with POLE-related disorders and has been described in the gnomAD database with an overall population frequency of 0.008% (dbSNP rs766511597). The p.Arg1508Cys change affects a moderately conserved amino acid residue located in a domain of the POLE protein that is not known to be functional. The p.Arg1508Cys substitution appears to be deleterious using several in-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL). Due to these contrasting evidences and the lack of functional studies, the clinical significance of the p.Arg1508Cys change remains unknown at this time.
