HCN4 (HGNC:16882): (hyperpolarization activated cyclic nucleotide gated potassium channel 4) This gene encodes a member of the hyperpolarization-activated cyclic nucleotide-gated potassium channels. The encoded protein shows slow kinetics of activation and inactivation, and is necessary for the cardiac pacemaking process. This channel may also mediate responses to sour stimuli. Mutations in this gene have been linked to sick sinus syndrome 2, also known as atrial fibrillation with bradyarrhythmia or familial sinus bradycardia. The p.Ala863Ser variant in the HCN4 gene has been previously reported in 1 individual with dilated cardiomyopathy (Yildirim et al., 2022). This variant was absent from large population databases, including the Genome Aggregation Database (http://gnomad.broadinstitute.org/). This variant is present in ClinVar (Variation ID: 573236). Computational tools predict that this variant does not impact protein function; however, the accuracy of in silico algorithms is limited. These data were assessed using the ACMG/AMP variant interpretation guidelines. In summary, the significance of the p.Ala863Ser variant is uncertain. This variant has not been reported in the literature in individuals with HCN4-related disease. This variant is not present in population databases (ExAC no frequency). This sequence change replaces alanine with serine at codon 863 of the HCN4 protein (p.Ala863Ser). The alanine residue is highly conserved and there is a moderate physicochemical difference between alanine and serine. -
