Variant summary: The PALB2 c.1463G>T (p.Ser488Ile) variant causes a missense change involving the alteration of a non-conserved nucleotide. 3/4 in silico tools predict a benign outcome for this variant (SNPsandGO not captured due to low reliability index). One study found non-significant LOH (FDR = 1%) for this variant for breast adenocarcinoma, where it was observed in both the tumor and the paired normal tissue types (Lu_Nature_2015). However, the exact significance of this finding towards an inherited predisposition to breast cancer is unclear. The variant of interest has been found in a large, broad control population, ExAC in 1/121390 control chromosomes at a frequency of 0.0000082, which does not exceed the estimated maximal expected allele frequency of a pathogenic PALB2 variant (0.0001563). In addition, one clinical diagnostic laboratory classified this variant as uncertain significance. The variant of interest has not, to our knowledge, been reported in affected individuals via publications and/or reputable databases/clinical diagnostic laboratories; nor evaluated for functional impact by in vivo/vitro studies. Because of the absence of clinical information and the lack of functional studies, the variant is classified as a variant of uncertain significance (VUS). -
The PALB2 c.1463G>T (p.Ser488Ile) variant has been reported in the published literature in at least one individual with breast cancer (PMID: 26689913 (2015)). The frequency of this variant in the general population, 0.00016 (4/24958 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

This missense variant replaces serine with isoleucine at codon 488 of the PALB2 protein. Computational prediction suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with breast cancer (PMID: 26689913). This variant has been identified in 4/282786 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.S488I variant (also known as c.1463G>T), located in coding exon 4 of the PALB2 gene, results from a G to T substitution at nucleotide position 1463. The serine at codon 488 is replaced by isoleucine, an amino acid with dissimilar properties. This alteration has been reported in a breast cancer patient from a cohort of 4034 cancer cases from The Cancer Genome Atlas (Lu C et al. Nat Commun. 2015 Dec;6:10086). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
