Variant summary: CSRP3 c.49G>A (p.Val17Ile) results in a conservative amino acid change located in the Zinc finger, LIM-type domain (IPR001781) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.2e-05 in 251444 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.49G>A has been reported in the literature in individuals affected with Preeclampsia (Gammill_2018). This report does not provide unequivocal conclusions about association of the variant with Cardiomyopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 30021846). Five ClinVar submitters have assessed the variant since 2014: all have classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.
The p.Val17Ile variant in CSRP3 has not been previously reported in individuals with cardiomyopathy, but has been identified in 3/126684 European chromosomes by the genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org/; dbS NP rs767932680). Computational prediction tools and conservation analysis sugges t that the p.Val17Ile variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In summary, the clinical sign ificance of the p.Val17Ile variant is uncertain.

This sequence change replaces valine, which is neutral and non-polar, with isoleucine, which is neutral and non-polar, at codon 17 of the CSRP3 protein (p.Val17Ile). This variant is present in population databases (rs767932680, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with CSRP3-related conditions. ClinVar contains an entry for this variant (Variation ID: 517386). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

This variant is reported in 0.012% of alleles in individuals of African descent in gnomAD (http://gnomad.broadinstitute.org/variant/11-19213947-C-T). The p.V17I variant (also known as c.49G>A), located in coding exon 1 of the CSRP3 gene, results from a G to A substitution at nucleotide position 49. The valine at codon 17 is replaced by isoleucine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
