RAD50 (HGNC:9816): (RAD50 double strand break repair protein) The protein encoded by this gene is highly similar to Saccharomyces cerevisiae Rad50, a protein involved in DNA double-strand break repair. This protein forms a complex with MRE11 and NBS1. The protein complex binds to DNA and displays numerous enzymatic activities that are required for nonhomologous joining of DNA ends. This protein, cooperating with its partners, is important for DNA double-strand break repair, cell cycle checkpoint activation, telomere maintenance, and meiotic recombination. Knockout studies of the mouse homolog suggest this gene is essential for cell growth and viability. Mutations in this gene are the cause of Nijmegen breakage syndrome-like disorder.[provided by RefSeq, Apr 2010]

The isoleucine at codon 94 is replaced by threonine, an amino acid with similar properties. In a study of 52 unrelated women considered at risk for hereditary breast and ovarian cancer (HBOC) who previously tested negative for BRCA1/2 mutations, this alteration was detected in one patient whose breast tumor showed loss of heterozygosity (Grasel RS et al. Front Oncol, 2020 Oct;10:571330). This alteration has also been reported in 1/1197 individuals from Greece, Romania, and Turkey undergoing evaluation for inherited cancer predisposition (Tsaousis GN et al. BMC Cancer, 2019 Jun;19:535). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This frequency is not higher than expected for a pathogenic variant in RAD50 causing Hereditary Breast And Ovarian Cancer Syndrome (4.4e-05 vs 6.3e-05), allowing no conclusion about variant significance. c.281T>C has been reported in the literature in one individual with personal of family history of breast and/or ovarian cancer (Tsaousis_2019). The report does not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three ClinVar submitters (evaluation after 2014) cite the variant as uncertain significance.
