The isoleucine at codon 94 is replaced by threonine, an amino acid with similar properties. In a study of 52 unrelated women considered at risk for hereditary breast and ovarian cancer (HBOC) who previously tested negative for BRCA1/2 mutations, this alteration was detected in one patient whose breast tumor showed loss of heterozygosity (Grasel RS et al. Front Oncol, 2020 Oct;10:571330). This alteration has also been reported in 1/1197 individuals from Greece, Romania, and Turkey undergoing evaluation for inherited cancer predisposition (Tsaousis GN et al. BMC Cancer, 2019 Jun;19:535). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |

This frequency is not higher than expected for a pathogenic variant in RAD50 causing Hereditary Breast And Ovarian Cancer Syndrome (4.4e-05 vs 6.3e-05), allowing no conclusion about variant significance. c.281T>C has been reported in the literature in one individual with personal of family history of breast and/or ovarian cancer (Tsaousis_2019). The report does not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome.
