The PALB2 c.3236C>T (p.Ala1079Val) variant has been reported in the published literature in individuals with breast cancer (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/)), colorectal cancer (PMID: 33309985 (2020)), and prostate cancer (PMID: 36045058 (2023)). This variant has also been observed in healthy control individuals (PMIDs: 30287823 (2018), 33309985 (2020), 33471991 (2021)). The frequency of this variant in the general population, 0.0000088 (1/113748 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

The p.A1079V variant (also known as c.3236C>T), located in coding exon 12 of the PALB2 gene, results from a C to T substitution at nucleotide position 3236. The alanine at codon 1079 is replaced by valine, an amino acid with similar properties. This alteration was not observed in unselected male breast cancer patients but was observed with an allele frequency of 0.0001 in 12490 male controls of Japanese ancestry. This variant was also identified in 2/12501 unselected Japanese colorectal cancer patients and in 2/23703 controls (Fujita M et al. Clin Gastroenterol Hepatol, 2022 Sep;20:2132-2141.e9). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

To our knowledge, functional studies have not been reported for this variant. This variant has been detected in a breast cancer case-control meta-analysis in 3/60463 cases and 1/53461 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID PALB2_010392) and in a separate breast cancer case-control study in two unaffected individuals and absent in 7051 female and 53 male breast cancer cases (PMID: 30287823). This variant also has been reported in a colorectal cancer case-control study in 2/12503 cases and 2/23705 unaffected individuals (PMID: 33309985) and in a pancreatic cancer case-control study in 2/23705 unaffected individuals and absent in 1005 cases (PMID: 32980694).
