The p.V2467I variant (also known as c.7399G>A), located in coding exon 49 of the ATM gene, results from a G to A substitution at nucleotide position 7399. The valine at codon 2467 is replaced by isoleucine, an amino acid with highly similar properties. This variant was detected in multiple individuals with a personal history of breast cancer (Hauke et al. Cancer Med. 2018 Apr;7:1349-1358; Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
This missense variant replaces valine with isoleucine at codon 2467 of the ATM protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. In a large international case-control study, this variant was reported in 1/60465 breast cancer cases and 0/53461 controls (PMID: 33471991). In another case-control study conducted in Germany, this variant was reported in 1/5589 breast cancer cases (PMID: 29522266). This variant has been identified in 2/251276 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The variant was identified in dbSNP (ID: rs769722643) as "With Uncertain significance allele", ClinVar (classified as uncertain significance by Invitae).The variant was identified in control databases in 2 of 246038 chromosomes at a frequency of 0.000008 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: Latino in 1 of 33578 chromosomes (freq: 0.00003), European Non-Finnish in 1 of 111528 chromosomes (freq: 0.000009), while the variant was not observed in the African, Other, Ashkenazi Jewish, East Asian, European Finnish, and South Asian populations. The p.Val2467 residue is conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact to the protein; this information is not very predictive of pathogenicity. The variant occurs outside of the splicing consensus sequence and 1 of 4 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity.
