ANK2 (HGNC:493): (ankyrin 2) This gene encodes a member of the ankyrin family of proteins that link the integral membrane proteins to the underlying spectrin-actin cytoskeleton. Ankyrins play key roles in activities such as cell motility, activation, proliferation, contact and the maintenance of specialized membrane domains. Most ankyrins are typically composed of three structural domains: an amino-terminal domain containing multiple ankyrin repeats; a central region with a highly conserved spectrin binding domain; and a carboxy-terminal regulatory domain which is the least conserved and subject to variation. The protein encoded by this gene is required for targeting and stability of Na/Ca exchanger 1 in cardiomyocytes. Mutations in this gene cause long QT syndrome 4 and cardiac arrhythmia syndrome. The ANK2 c.11525G>A; p.Arg3842Gln variant (rs770954138), to our knowledge, is not reported in the medical literature but is reported in ClinVar (Variation ID: 234767). This variant is only observed on eight alleles in the Genome Aggregation Database, indicating it is not a common polymorphism. The arginine at codon 3842 is moderately conserved, and computational analyses (SIFT, PolyPhen-2) predict that this variant is tolerated. Due to limited information, the clinical significance of the p.Arg3842Gln variant is uncertain at this time. -

A variant of uncertain significance has been identified in the ANK2 gene. The R3842Q variant has not been published as pathogenic or reported as benign to our knowledge. This variant is not observed at a significant frequency in large population cohorts (Lek et al., 2016). The R3842Q variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. However, in-silico analyses, including protein predictors and evolutionary conservation, support that this variant does not alter protein structure/function. Finally, while missense variants in nearby residues (S3839T, T3844N) have been reported in the Human Gene Mutation Database in association with LQTS (Stenson et al., 2014), the pathogenicity of these variants has not been definitively determined. -
