The c.1182_1184delTGT variant (also known as p.V395del) is located in coding exon 7 of the MYLK gene. This variant results from an in-frame TGT deletion at nucleotide positions 1182 to 1184. This results in the in-frame deletion of a valine at codon 395. This variant has been detected in an individual with acute aortic dissection (Zheng J et al. Int. J. Legal Med., 2018 Sep;132:1273-1280). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be inconclusive by in silico analysis (Choi Y et al. PLoS ONE. 2012; 7(10):e46688). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: MYLK c.1182_1184delTGT (p.Val395del) results in an in-frame deletion that is predicted to remove one amino acid from the encoded protein. The variant allele was found at a frequency of 0.00012 in 252492 control chromosomes, predominantly at a frequency of 0.0013 within the East Asian subpopulation in the gnomAD database. The observed variant frequency within East Asian control individuals in the gnomAD database is approximately 26 fold of the estimated maximal expected allele frequency for a pathogenic variant in MYLK causing Thoracic Aortic Aneurysms And Dissections phenotype (5e-05), strongly suggesting that the variant is a benign polymorphism found primarily in populations of East Asian origin. c.1182_1184delTGT has been reported in the literature in one individual affected with Acute aortic dissection (Zheng_2018). The report does not provide unequivocal conclusions about association of the variant with Thoracic Aortic Aneurysms And Dissections. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. One ClinVar submitter (evaluation after 2014) cites the variant as likely benign. Based on the evidence outlined above, the variant was classified as likely benign. -
