The p.R1112W variant (also known as c.3334A>T), located in coding exon 21 of the RAD50 gene, results from an A to T substitution at nucleotide position 3334. The arginine at codon 1112 is replaced by tryptophan, an amino acid with dissimilar properties. This variant was detected in 1/1824 patients with triple-negative breast cancer unselected for family history (Couch FJ et al. J. Clin. Oncol. 2015 Feb;33:304-11). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 1112 of the RAD50 protein (p.Arg1112Trp). This variant is present in population databases (rs773047090, gnomAD 0.0009%). This missense change has been observed in individual(s) with breast cancer (PMID: 25452441). ClinVar contains an entry for this variant (Variation ID: 187670). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RAD50 protein function with a positive predictive value of 80%.
