Variant summary

Our verdict is Pathogenic. The variant received 16 ACMG points: 16P and 0B. PVS1PS3PM2PP5_Moderate

The NM_000477.7(ALB):c.714-2A>G variant causes a splice acceptor, intron change. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 3/3 splice prediction tools predicting alterations to normal splicing. Variant has been reported in ClinVar as Pathogenic (★). ClinVar reports functional evidence for this variant: "SCV004170213: Published functional studies demonstrate a damaging effect, specifically ligation of the exon 6 and 7 sequences (Ruffner et al., 1988)" and additional evidence is available in ClinVar.

Genes affected
ALB (HGNC:399): (albumin) This gene encodes the most abundant protein in human blood. This protein functions in the regulation of blood plasma colloid osmotic pressure and acts as a carrier protein for a wide range of endogenous molecules including hormones, fatty acids, and metabolites, as well as exogenous drugs. Additionally, this protein exhibits an esterase-like activity with broad substrate specificity. The encoded preproprotein is proteolytically processed to generate the mature protein. A peptide derived from this protein, EPI-X4, is an endogenous inhibitor of the CXCR4 chemokine receptor. [provided by RefSeq, Jul 2016]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 16 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, LoF is a know mechanism of disease, Cryptic splice site detected, with MaxEntScore 4.2, offset of 5, new splice context is: cgattttctttttggggcAGtag. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in frameshift change.
PS3
PS3 evidence extracted from ClinVar submissions: SCV004170213: Published functional studies demonstrate a damaging effect, specifically ligation of the exon 6 and 7 sequences (Ruffner et al., 1988); PMID: 3353369, 22227324, 32553838
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 4-73411994-A-G is Pathogenic according to our data. Variant chr4-73411994-A-G is described in ClinVar as Pathogenic. ClinVar VariationId is 18210.Status of the report is criteria_provided_single_submitter, 1 stars.
