Two of four in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 249196 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.14302G>A has been reported in the literature in one individual affected with Catecholaminergic Polymorphic Ventricular Tachycardia (Sarquella-Brugada_2021). The report does not provide unequivocal conclusions about association of the variant with Catecholaminergic Polymorphic Ventricular Tachycardia. The p.V4768I variant (also known as c.14302G>A), located in coding exon 100 of the RYR2 gene, results from a G to A substitution at nucleotide position 14302. The valine at codon 4768 is replaced by isoleucine, an amino acid with highly similar properties. This alteration has been reported in an inherited channelopathy cohort (Sarquella-Brugada G et al. Hum Genet, 2021 Sep;[ePub ahead of print]). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
