Variant summary

Our verdict is Pathogenic. Variant got 11 ACMG points: 11P and 0B. PM1PM2PM5PP2PP3_ModeratePP5_Moderate

The NM_001370259.2(MEN1):c.1355G>T(p.Arg452Leu) variant causes a missense change. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R452P) has been classified as Likely pathogenic.

Genes affected
MEN1 (HGNC:7010): (menin 1) This gene encodes menin, a tumor suppressor associated with a syndrome known as multiple endocrine neoplasia type 1. Menin is a scaffold protein that functions in histone modification and epigenetic gene regulation. It is thought to regulate several pathways and processes by altering chromatin structure through the modification of histones. [provided by RefSeq, May 2019]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 11 ACMG points.

PM1
In a hotspot region, there are 3 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 1 benign, 15 uncertain in NM_001370259.2
PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chr11-64804813-G-C is described in ClinVar as [Likely_pathogenic]. Clinvar id is 2987184.Status of the report is criteria_provided_single_submitter, 1 stars.
PP2
Missense variant where missense usually causes diseases, MEN1
PP3
MetaRNN computational evidence supports a deleterious effect, 0.926
PP5
Variant 11-64804812-C-A is Pathogenic according to our data. Variant chr11-64804812-C-A is described in ClinVar as [Likely_pathogenic]. Clinvar id is 2564967.Status of the report is criteria_provided_single_submitter, 1 stars.

Likely pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsMay 11, 2023The p.R452L variant (also known as c.1355G>T), located in coding exon 9 of the MEN1 gene, results from a G to T substitution at nucleotide position 1355. The arginine at codon 452 is replaced by leucine, an amino acid with dissimilar properties. This alteration has been observed in at least one individual with a personal and/or family history that is consistent with MEN1-related disease (Ambry internal data). Another alteration at the same codon, p.R452W (c.1354C>T), has been reported in multiple families with clinical features of MEN1 (Pardi E et al. PLoS ONE 2017 Oct;12(10):e0186485; Ambry internal data), and is predicted to have a damaging impact on protein structure (Shi A et al. Blood, 2012 Nov;120:4461-9; Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
