Variant summary
Our verdict is Pathogenic. The variant received 14 ACMG points: 14P and 0B. PM5PP3_StrongPP5_Very_Strong
The NM_016035.5(COQ4):c.370G>A(p.Gly124Ser) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000167 in 1,614,054 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G124V) has been classified as Pathogenic.

Neonatal encephalomyopathy-cardiomyopathy-respiratory distress syndrome Pathogenic:3
This variant was determined to be pathogenic according to ACMG Guidelines, 2015 [PMID:25741868]. -
This sequence change replaces glycine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 124 of the COQ4 protein (p.Gly124Ser). This variant is present in population databases (rs776825296, gnomAD 0.2%). This missense change has been observed in individual(s) with clinical features of coenzyme Q10 deficiency (PMID: 30659264). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 476179). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on COQ4 protein function. Experimental studies have shown that this missense change affects COQ4 function (PMID: 30659264). For these reasons, this variant has been classified as Pathogenic. -
