This sequence change replaces glutamine, which is neutral and polar, with histidine, which is basic and polar, at codon 303 of the POLE protein (p.Gln303His). This variant also falls at the last nucleotide of exon 9, which is part of the consensus splice site for this exon. This variant is present in population databases (rs779801916, gnomAD 0.05%). This variant has not been reported in the literature in individuals affected with POLE-related conditions. ClinVar contains an entry for this variant (Variation ID: 560852). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.Q303H variant (also known as c.909G>C), located in coding exon 9 of the POLE gene, results from a G to C substitution at nucleotide position 909. The glutamine at codon 303 is replaced by histidine, an amino acid with highly similar properties. However, this change occurs in the last base pair of coding exon 9, which makes it likely to have some effect on normal mRNA splicing. This nucleotide position is well conserved in available vertebrate species. This amino acid position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration may weaken the native splice donor site and will result in the creation or strengthening of a novel splice donor site. In addition, as a missense substitution the in silico prediction for this alteration is inconclusive. Loss of function of POLE has not been established as a mechanism of disease. Based on the available evidence, the clinical significance of this alteration remains unclear. -
