MYO6 (HGNC:7605): (myosin VI) This gene encodes a reverse-direction motor protein that moves toward the minus end of actin filaments and plays a role in intracellular vesicle and organelle transport. The protein consists of a motor domain containing an ATP- and an actin-binding site and a globular tail which interacts with other proteins. This protein maintains the structural integrity of inner ear hair cells and mutations in this gene cause non-syndromic autosomal dominant and recessive hearing loss. This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 946 of the MYO6 protein (p.Arg946His). This variant is present in population databases (rs781754117, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with MYO6-related conditions. ClinVar contains an entry for this variant (Variation ID: 504988). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt MYO6 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.Arg946His var iant in MYO6 has not been previously reported in individuals with hearing loss, but has been identified in 1/66074 European chromosomes by the Exome Aggregation Consortium (ExAC, http://exac.broadinstitute.org; dbSNP rs781754117). Arginine (Arg) at position 946 is not conserved in mammals or evolutionarily distant spec ies and two mammals (naked mole rat and hedgehog) carry a Histidine (His), raisi ng the possibility that this change may be tolerated. Additional computational prediction tools suggest that the p.Arg946His variant may not impact the protein , though this information is not predictive enough to rule out pathogenicity. In summary, while the clinical significance of the p.Arg946His variant is uncertai n, these data suggest that it is more likely to be benign. -
