The arginine at codon 169 is replaced by cysteine, an amino acid with highly dissimilar properties. This alteration has been reported in the homozygous state in multiple unrelated patients with a phenotype consistent with Brown-Vialetto-Van Laere syndrome (Allison T et al. J Child Neurol, 2017 05;32:528-532; Pasquini E et al. Acta Myol, 2017 06;36:47-119; Woodcock IR et al. Semin Pediatr Neurol, 2018 07;26:2-9; Sun M et al. Genet Med, 2019 01;21:195-206). Based on data from gnomAD, this allele has an overall frequency of 0.0020% (5/250098) total alleles studied. The highest observed frequency was 0.0035% (4/112704) of non-Finnish European alleles. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis.
