Hereditary cancer-predisposing syndrome;C4707243:Familial thoracic aortic aneurysm and aortic dissection Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Aug 14, 2023 | The p.V465M variant (also known as c.1393G>A), located in coding exon 10 of the SMAD4 gene, results from a G to A substitution at nucleotide position 1393. The valine at codon 465 is replaced by methionine, an amino acid with highly similar properties. This alteration has been reported in one individual with Juvenile Polyposis syndrome. BMP signaling studies showed reduced luciferase activity; however, the reduction was not statistically significant from that found for the wild-type vector (Carr JC et al. J. Surg. Res. 2012 May;174:211-4).This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
Hereditary cancer-predisposing syndrome Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Color Diagnostics, LLC DBA Color Health | Aug 07, 2019 | This missense variant replaces valine with methionine at codon 465 of the SMAD4 protein. Computational prediction tools and conservation analyses suggest that this variant may have deleterious impact on the protein function. Computational splicing tools suggest that this variant may not impact RNA splicing. A functional study has shown that the variant causes 30% reduction in BMP signaling, but the difference is not statistically significant from that in the wild-type (PMID: 22316667). This variant has been reported in an individual affected with juvenile polyposis (PMID: 22316667). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. - |

Juvenile polyposis syndrome Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Dec 01, 2023 | This sequence change replaces valine, which is neutral and non-polar, with methionine, which is neutral and non-polar, at codon 465 of the SMAD4 protein (p.Val465Met). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with juvenile polyposis (PMID: 22316667). ClinVar contains an entry for this variant (Variation ID: 184929). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on SMAD4 protein function. Experimental studies are conflicting or provide insufficient evidence to determine the effect of this variant on SMAD4 function (PMID: 22316667). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. - |
