However, this variant has been reported in an individual with breast cancer (PMID: 25186627 (2015)). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. The p.I58V variant (also known as c.172A>G), located in coding exon 1 of the SMAD4 gene, results from an A to G substitution at nucleotide position 172. The isoleucine at codon 58 is replaced by valine, an amino acid with highly similar properties. This variant was reported in 1/1781 individuals who were referred for BRCA1/2 gene testing and underwent multigene panel testing (Tung N et al. Cancer. 2015 Jan;121:25-33). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
