ATP7B (HGNC:870): (ATPase copper transporting beta) This gene is a member of the P-type cation transport ATPase family and encodes a protein with several membrane-spanning domains, an ATPase consensus sequence, a hinge domain, a phosphorylation site, and at least 2 putative copper-binding sites. This protein is a monomer, and functions as a copper-transporting ATPase which exports copper out of the cells, such as the efflux of hepatic copper into the bile. Alternate transcriptional splice variants, encoding different isoforms with distinct cellular localizations, have been characterized. Mutations in this gene have been associated with Wilson disease which is characterized by copper accumulation. [provided by RefSeq, Dec 2019]

Variant summary: ATP7B c.254G>T (p.Gly85Val) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 249242 control chromosomes (gnomAD). c.254G>T has been observed in individuals affected with Wilson Disease (e.g., Loudianos_1998, Sanchez-Monteagudo_2020, Zhang_2022, Gorukmez_2023). These data indicate that the variant is likely to be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function, finding that the variant results in <10% of normal copper uptake activity as well as severely reduced phosphorylation activity (e.g., Huster_2012). The following publications have been ascertained in the context of this evaluation (PMID: 22240481, 9671269, 32043565, 35220961). ClinVar contains an entry for this variant (Variation ID: 189037). Based on the evidence outlined above, the variant was classified as pathogenic.

This sequence change replaces glycine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 85 of the ATP7B protein (p.Gly85Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with Wilson disease (PMID: 30702195, 32043565). ClinVar contains an entry for this variant (Variation ID: 189037). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ATP7B protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects ATP7B function (PMID: 17919502, 30702195). This variant disrupts the p.Gly85 amino acid residue in ATP7B. Other variant(s) that disrupt this residue have been determined to be pathogenic (Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
