Our verdict is Likely pathogenic. Variant got 8 ACMG points: 8P and 0B. PM2PP3_StrongPP5_Moderate
The NM_000918.4(P4HB):c.1178A>G(p.Tyr393Cys) variant causes a missense, splice region change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 13/23 in silico tools predict a damaging outcome for this variant. 3/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Pathogenic (★).
P4HB (HGNC:8548): (prolyl 4-hydroxylase subunit beta) This gene encodes the beta subunit of prolyl 4-hydroxylase, a highly abundant multifunctional enzyme that belongs to the protein disulfide isomerase family. When present as a tetramer consisting of two alpha and two beta subunits, this enzyme is involved in hydroxylation of prolyl residues in preprocollagen. This enzyme is also a disulfide isomerase containing two thioredoxin domains that catalyze the formation, breakage and rearrangement of disulfide bonds. Other known functions include its ability to act as a chaperone that inhibits aggregation of misfolded proteins in a concentration-dependent manner, its ability to bind thyroid hormone, its role in both the influx and efflux of S-nitrosothiol-bound nitric oxide, and its function as a subunit of the microsomal triglyceride transfer protein complex. Variant 17-81845742-T-C is Pathogenic according to our data. Variant chr17-81845742-T-C is described in ClinVar as [Pathogenic]. Clinvar id is 189337.Status of the report is criteria_provided_single_submitter, 1 stars. Variant chr17-81845742-T-C is described in Lovd as [Pathogenic].
This sequence change replaces tyrosine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 393 of the P4HB protein (p.Tyr393Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with Cole-Carpenter syndrome (PMID: 25683117, 29263160, 30063094). ClinVar contains an entry for this variant (Variation ID: 189337). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects P4HB function (PMID: 25683117). For these reasons, this variant has been classified as Pathogenic. -
