Variant summary: ABCC8 c.4591A>G (p.Thr1531Ala) results in a non-conservative amino acid change located in the ABC transporter-type domain (IPR003439) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251358 control chromosomes. c.4591A>G has been reported in the literature along with different second pathogenic variants in two individuals affected with Hyperinsulinism (Ellard_2013, Kapoor_2013, Sandal_2009). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 23771172, 23345197, 19475716). ClinVar contains an entry for this variant (Variation ID: 1338651). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -
DNA sequence analysis of the ABCC8 gene demonstrated a sequence change, c.4591A>G, in exon 38 that results in an amino acid change, p.Thr1531Ala. This sequence change has not been described in population database including gnomAD. The p.Thr1531Ala change affects a highly conserved amino acid residue located in a domain of the ABCC8 protein that is known to be functional. In-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL) provide contradictory results for the p.Thr1531Ala substitution. This sequence change has previously been reported in the compound heterozygous state with another ABCC8 variant in an individual with congenital hyperinsulinism (PMID: 19475716). Due to insufficient evidences and the lack of functional studies, the clinical significance of the p.Thr1531Ala change remains unknown at this time. -
