The variant is rare in population databases (gnomAD allele frequency = 0.0052%; 8 het and 0 hom in 152162 sequenced alleles; highest frequency = 0.0073%, Non-Finnish European population) (PM2_supporting). This variant is a novel missense change at an amino acid residue where a different missense change has been seen before (p.Arg1086His on ClinVar as drug response/pathogenic in Malignant hyperthermia c.3256C>A; p.Arg1086Ser on ClinVar as likely path) (PM5). Variant summary: CACNA1S c.3256C>T (p.Arg1086Cys) results in a non-conservative amino acid change in the encoded protein sequence and alters the first nucleotide of exon 26 adjacent to the intron 25 splice acceptor site. Five of five in-silico tools predict a damaging effect of the variant on protein function. 4/4 computational tools predict no significant impact on normal splicing. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 1.6e-05 in 251428 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.3256C>T has been reported in the literature in at least one individual affected with Malignant Hyperthermia Susceptibility (e.g. Jurkat-Rott_2000). These data do not allow any conclusion about variant significance. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 25637381, 10590402). Four submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.

This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 1086 of the CACNA1S protein (p.Arg1086Cys). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with malignant hyperthermia (PMID: 10590402). ClinVar contains an entry for this variant (Variation ID: 21035). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. This variant disrupts the p.Arg1086 amino acid residue in CACNA1S. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 9199552, 11260227, 12411788, 20431982). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing.
