The amino acid Ile at position 556 is changed to a Asn changing protein sequence and it might alter its composition and physico-chemical properties. This variant has been reported to the ClinVar database as Likely Pathogenic. The variant is predicted to be damaging by both SIFT and PolyPhen2. The residue is conserved across species. The amino acid change p.Ile556Asn in CLCN1 is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. This sequence change replaces isoleucine with asparagine at codon 556 of the CLCN1 protein (p.Ile556Asn). The isoleucine residue is highly conserved and there is a large physicochemical difference between isoleucine and asparagine. This variant is present in population databases (rs80356697, ExAC 0.01%). This variant has been observed in the homozygous state in individuals affected with myotonia congenita as well as in the heterozygous state in more mildly affected and clinically unaffected individuals (Plassart-Schiess E et al, Arzel-Hézode M et al).This variant has been reported to affect CLCN1 protein function (Saviane C et al, Kubisch C et al). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. This variant is present in population databases (rs80356697, gnomAD 0.02%). This missense change has been observed in individuals with autosomal recessive myotonia (PMID: 9566422, 19882638, 31069529; Invitae). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 21041). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CLCN1 protein function.
