Variant summary: BRCA1 c.3759dupT (p.Lys1254X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. The variant was absent in 251272 control chromosomes. c.3759dupT has been reported in the literature in multiple individuals affected with Hereditary Breast And Ovarian Cancer Syndrome (e.g. Judkins_2005, van Orsouw_1999). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 16267036, 10528853). ClinVar contains an entry for this variant (Variation ID: 54992). Loss of function of the BRCA1 gene is an established disease mechanism in autosomal dominant hereditary breast and ovarian cancer syndrome (HBOC). Additionally, this variant was classified as Pathogenic on Sept 8, 2016 by the ClinGen-approved ENIGMA expert panel (Variation ID: 54992). The c.3759dupT (p.K1254*) alteration, located in exon 10 (coding exon 9) of the BRCA1 gene, consists of a duplication of T at position 3759, causing a translational frameshift with a predicted alternate stop codon after amino acids. This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This variant has been reported in multiple individuals and families with breast and/or ovarian cancer (Greenman, 1998; van Orsouw, 1999; de Juan Jiménez, 2013; Alvarez, 2017). Of note, this variant is also designated as 3878insT, 3879insT, and c.3759_3760insT in some published literature. Based on the available evidence, this alteration is classified as pathogenic. -
