This missense variant replaces tyrosine with histidine at codon 761 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in one individual each affected with pancreatic cancer and breast cancer (PMID: 31432501, 33471991; Leiden Open Variation Database DB-ID BRCA2_002347). This variant has been identified in 1/251178 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.Y761H variant (also known as c.2281T>C), located in coding exon 10 of the BRCA2 gene, results from a T to C substitution at nucleotide position 2281. The tyrosine at codon 761 is replaced by histidine, an amino acid with similar properties. This alteration was identified in a French individual diagnosed with pancreatic cancer (Schwartz M et al. Clin Genet, 2019 12;96:579-584). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

The BRCA2 c.2281T>C (p.Tyr761His) variant has been reported in the published literature in an individual with pancreatic cancer (PMID: 31432501 (2019)) and is described to be located in a region of the BRCA2 gene that is tolerant to missense sequence changes (PMID: 31911673 (2020)). In a large breast cancer association study, this variant was found in both a healthy individual and an individual with breast cancer (PMID: 33471991 (2021), see also LOVD (https://databases.lovd.nl/shared/variants/BRCA2)). The frequency of this variant in the general population, 0.000004 (1/251178 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -
Not observed at a significant frequency in large population cohorts (Lek 2016); In silico analysis, which includes protein predictors and evolutionary conservation, supports that this variant does not alter protein structure/function; Has not been previously published as pathogenic or benign to our knowledge; This variant is associated with the following publications: (PMID: 31432501) -
Hereditary breast ovarian cancer syndrome Uncertain:1
This sequence change replaces tyrosine, which is neutral and polar, with histidine, which is basic and polar, at codon 761 of the BRCA2 protein (p.Tyr761His). This variant is present in population databases (rs80358499, gnomAD 0.0009%). This missense change has been observed in individual(s) with pancreatic cancer and breast cancer (PMID: 31432501, 38074417). ClinVar contains an entry for this variant (Variation ID: 37779). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
