Missense variant in a coldspot region where missense variants are very unlikely to be pathogenic (PMID:31911673). -
This missense variant replaces glutamic acid with glycine at codon 1046 of the BRCA2 protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in individuals affected with ovarian cancer and pancreatic cancer (PMID: 16284991, Color internal data). This variant has been identified in 1/246562 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.E1046G variant (also known as c.3137A>G), located in coding exon 10 of the BRCA2 gene, results from an A to G substitution at nucleotide position 3137. The glutamic acid at codon 1046 is replaced by glycine, an amino acid with similar properties. This alteration has been reported as a variant of unknown significance in an individual diagnosed with ovarian cancer (Pal T et al. Cancer. 2005 Dec; 104(12):2807-16). This variant has also been reported in 1/1120 pediatric cancer patients who underwent whole genome sequencing and/or whole exome sequencing; this patient was diagnosed with low grade glioma (Zhang J et al. N Engl J Med, 2015 Dec;373:2336-2346). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

The BRCA2 c.3137A>G (p.Glu1046Gly) variant has been reported in the published literature in individuals with a personal and/or family history of breast and/or ovarian cancer (PMID: 34284872 (2022), 16284991 (2005)), and described to be located in a region of the BRCA2 gene that is tolerant to missense sequence changes (PMID: 31911673 (2020)). The frequency of this variant in the general population, 0.0000041 (1/246562 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.3e-05 in 1609298 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in BRCA2 causing Hereditary Breast And Ovarian Cancer Syndrome (1.3e-05 vs 0.00075), allowing no conclusion about variant significance. c.3137A>G has been reported in the literature in individuals affected with ovarian carcinoma (example, Pal_2005, Krivokuca_2022) and in a patient with a low grade glioma (example, Zhang_2015). These report(s) do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. At least one publication reports experimental evidence evaluating an impact on protein function. These results showed no damaging effect of this variant (Biswas_2023).
