The variant was identified in dbSNP (ID: rs80358669) as "With Uncertain significance allele", ClinVar (classified as uncertain significance by Invitae, Ambry Genetics, and BIC), MutDB, LOVD 3.0, UMD-LSDB (1x as unclassified variant), and in BIC (1x as unknown significance). The variant was identified in control databases in 2 of 238558 chromosomes at a frequency of 0.000008 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: European in 1 of 109132 chromosomes (freq: 0.000009) and South Asian in 1 of 28378 chromosomes (freq: 0.00004), while the variant was not observed in the African, Other, Latino, Ashkenazi Jewish, East Asian, or Finnish populations. The p.Lys1440 residue is conserved in mammals but not in more distantly related organisms and computational analyses (SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; this information is not predictive enough to rule out pathogenicity. Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
The p.K1440R variant (also known as c.4319A>G), located in coding exon 10 of the BRCA2 gene, results from an A to G substitution at nucleotide position 4319. The lysine at codon 1440 is replaced by arginine, an amino acid with highly similar properties. Functional analysis of this variant in a BRCA2 ortholog revealed results largely similar to wildtype BRCA2, but some impact on RAD51 recruitment was observed (Yoshikawa Y et al. PLoS ONE, 2012;7:e45833). This variant was also observed in 1/3251 individuals who met eligibility criteria for hereditary breast and ovarian cancer syndrome. The individual was diagnosed with ovarian cancer at age 38 (Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This variant is present in population databases (rs80358669, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with BRCA2-related conditions. ClinVar contains an entry for this variant (Variation ID: 51632). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
