Genes affected
BRCA2 (HGNC:1101): (BRCA2 DNA repair associated) Inherited mutations in BRCA1 and this gene, BRCA2, confer increased lifetime risk of developing breast or ovarian cancer. Both BRCA1 and BRCA2 are involved in maintenance of genome stability, specifically the homologous recombination pathway for double-strand DNA repair. The largest exon in both genes is exon 11, which harbors the most important and frequent mutations in breast cancer patients. The BRCA2 gene was found on chromosome 13q12.3 in human. The BRCA2 protein contains several copies of a 70 aa motif called the BRC motif, and these motifs mediate binding to the RAD51 recombinase which functions in DNA repair. BRCA2 is considered a tumor suppressor gene, as tumors with BRCA2 mutations generally exhibit loss of heterozygosity (LOH) of the wild-type allele. [provided by RefSeq, May 2020]

Uncertain significance, no assertion criteria providedclinical testingDepartment of Pathology and Laboratory Medicine, Sinai Health System-The BRCA2 p.Lys1440Arg variant was not identified in the literature nor was it identified in GeneInsight-COGR, Cosmic, ARUP Laboratories, or Zhejiang University databases. The variant was identified in dbSNP (ID: rs80358669) as "With Uncertain significance allele", ClinVar (classified as uncertain significance by Invitae, Ambry Genetics, and BIC), MutDB, LOVD 3.0, UMD-LSDB (1x as unclassified variant), and in BIC (1x as unknown significance). The variant was identified in control databases in 2 of 238558 chromosomes at a frequency of 0.000008 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: European in 1 of 109132 chromosomes (freq: 0.000009) and South Asian in 1 of 28378 chromosomes (freq: 0.00004), while the variant was not observed in the African, Other, Latino, Ashkenazi Jewish, East Asian, or Finnish populations. The p.Lys1440 residue is conserved in mammals but not in more distantly related organisms and computational analyses (SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance. -
Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsApr 12, 2022The p.K1440R variant (also known as c.4319A>G), located in coding exon 10 of the BRCA2 gene, results from an A to G substitution at nucleotide position 4319. The lysine at codon 1440 is replaced by arginine, an amino acid with highly similar properties. Functional analysis of this variant in a BRCA2 ortholog revealed results largely similar to wildtype BRCA2, but some impact on RAD51 recruitment was observed (Yoshikawa Y et al. PLoS ONE, 2012;7:e45833). This variant was also observed in 1/3251 individuals who met eligibility criteria for hereditary breast and ovarian cancer syndrome. The individual was diagnosed with ovarian cancer at age 38 (Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Hereditary breast ovarian cancer syndrome Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingInvitaeFeb 08, 2018In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies. This variant has been reported in individuals affected with breast cancer in the Leiden Open-source Variation Database (PMID: 21520333). ClinVar contains an entry for this variant (Variation ID: 51632). This variant is present in population databases (rs80358669, ExAC 0.002%). This sequence change replaces lysine with arginine at codon 1440 of the BRCA2 protein (p.Lys1440Arg). The lysine residue is moderately conserved and there is a small physicochemical difference between lysine and arginine. -
