This missense variant replaces methionine with valine at codon 2393 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Splice site prediction tools suggest that this variant may impact RNA splicing by the creation of a new donor site, however this effect on splicing appears to affect less than 5% of transcripts (PMID 31191615). To our knowledge, protein functional studies have not been performed for this variant. This variant has been reported in a multifactorial analysis with co-occurrence and family history likelihood ratios for pathogenicity of 1.0499 and 0.4781, respectively (PMID: 31131967). This variant has been identified in 2/251284 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The BRCA2 c.7177A>G (p.Met2393Val) variant has been reported in the published literature in a screen of high-risk breast and/or ovarian cancer families (PMID: 22476429 (2012)) as well as in a reportedly healthy individual (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared)). A minigene splicing study has shown that this variant results in aberrant splicing. However, these aberrant transcripts are expressed at a very low level and the predominant transcript expressed is the full length BRCA2 transcript (PMID: 31191615 (2019)). The frequency of this variant in the general population, 0.000008 (2/251284 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

The p.M2393V variant (also known as c.7177A>G), located in coding exon 13 of the BRCA2 gene, results from an A to G substitution at nucleotide position 7177. The methionine at codon 2393 is replaced by valine, an amino acid with highly similar properties. This amino acid position is poorly conserved in available vertebrate species. This alteration has been shown to result in low-level aberrant RNA splicing (Fraile-Bethencourt E et al. Front Genet, 2019 May;10:503). RNA studies have demonstrated that this alteration does not result in abnormal splicing in the set of samples tested (Ambry internal data). In addition, this alteration is predicted to be tolerated by protein in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
Hereditary breast ovarian cancer syndrome Uncertain:1
This sequence change replaces methionine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 2393 of the BRCA2 protein (p.Met2393Val). This variant is present in population databases (rs80358945, gnomAD 0.003%). This missense change has been observed in individual(s) with clinical features of BRCA2-related conditions (PMID: 10923033). ClinVar contains an entry for this variant (Variation ID: 52277). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (internal data). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
