Breast-ovarian cancer, familial, susceptibility to, 2 Uncertain:3
This missense variant replaces histidine with arginine at codon 2548 of the BRCA2 protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in a breast cancer case-control meta-analysis in 1/60465 cases and 3/53458 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID BRCA2_003368). This variant has been identified in 1/251018 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -

Variant summary: BRCA2 c.7643A>G (p.His2548Arg) results in a non-conservative amino acid change located in the helical domain (IPR015252) of the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251018 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.7643A>G in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome and no experimental evidence demonstrating its impact on protein function have been reported. The following publications have been ascertained in the context of this evaluation (PMID: 19043619, 25348012). ClinVar contains an entry for this variant (Variation ID: 38112). Based on the evidence outlined above, the variant was classified as uncertain significance. -
BRCA2-related disorder Uncertain:1
The BRCA2 c.7643A>G variant is predicted to result in the amino acid substitution p.His2548Arg. This variant has been reported in an individual with breast cancer but was also detected in three non-cancer controls in a large breast cancer case-control study (Supplemental Data, Breast Cancer Association Consortium et al. 2021. PubMed ID: 33471991). This variant is reported in 0.00088% of alleles in individuals of European (non-Finnish) descent in gnomAD and is interpreted as uncertain in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/38112/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
not provided Uncertain:1
In silico analysis supports that this missense variant does not alter protein structure/function; Not observed at significant frequency in large population cohorts (gnomAD); Observed in breast cancer patients but also in unaffected controls (Dorling et al., 2021); Also known as 7871A>G; This variant is associated with the following publications: (PMID: 19043619, 25348012, 10923033, 32377563, 12228710, 29884841, 31131967, 33471991, 29881398) -

Hereditary breast ovarian cancer syndrome Uncertain:1
This sequence change replaces histidine, which is basic and polar, with arginine, which is basic and polar, at codon 2548 of the BRCA2 protein (p.His2548Arg). This variant is present in population databases (rs80358992, gnomAD 0.0009%). This missense change has been observed in individual(s) with clinical features of BRCA2-related conditions (PMID: 10923033). ClinVar contains an entry for this variant (Variation ID: 38112). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
