Variant summary
Our verdict is Pathogenic. The variant received 10 ACMG points: 10P and 0B. PS1PM1PM2PP3PP5
The NM_000059.4(BRCA2):c.7819A>C(p.Thr2607Pro) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Likely pathogenic in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. T2607A) has been classified as Uncertain significance.

Hereditary cancer-predisposing syndrome Pathogenic:1
The p.T2607P variant (also known as c.7819A>C), located in coding exon 16 of the BRCA2 gene, results from an A to C substitution at nucleotide position 7819. The threonine at codon 2607 is replaced by proline, an amino acid with highly similar properties. Using a computational method that produces a probabilistic likelihood ratio predictive of whether a missense variant impairs protein function, this alteration is predicted to be likely deleterious (Karchin R et al. Cancer Inform. 2008;6:203-16). This alteration was classified as likely pathogenic based on a multifactorial analysis model of variant classification (Parsons MT et al. Hum Mutat, 2019 09;40:1557-1578). Additionally, this alteration was non-functional in a homology directed DNA repair assay (Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -

Hereditary breast ovarian cancer syndrome Uncertain:1
In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Experimental studies have shown that this missense change affects BRCA2 function (PMID: 32444794). Based on a multifactorial likelihood algorithm using genetic, in silico, and/or statistical data, this variant has been determined to have a high probability of being pathogenic (PMID: 31131967, 31294896). ClinVar contains an entry for this variant (Variation ID: 52422). This variant has not been reported in the literature in individuals affected with BRCA2-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces threonine, which is neutral and polar, with proline, which is neutral and non-polar, at codon 2607 of the BRCA2 protein (p.Thr2607Pro). -
