FH (HGNC:3700): (fumarate hydratase) The protein encoded by this gene is an enzymatic component of the tricarboxylic acid (TCA) cycle, or Krebs cycle, and catalyzes the formation of L-malate from fumarate. It exists in both a cytosolic form and an N-terminal extended form, differing only in the translation start site used. The N-terminal extended form is targeted to the mitochondrion, where the removal of the extension generates the same form as in the cytoplasm. It is similar to some thermostable class II fumarases and functions as a homotetramer. Mutations in this gene can cause fumarase deficiency and lead to progressive encephalopathy. The p.Q47* pathogenic mutation (also known as c.139C>T), located in coding exon 2 of the FH gene, results from a C to T substitution at nucleotide position 139. This changes the amino acid from a glutamine to a stop codon within coding exon 2. This mutation was identified in a case report of a 30-year-old woman with cutaneous leiomyomas, uterine myomata, a renal cyst, and an adrenal adenoma (Tulandi T et al. CMAJ, 2016 Feb;188:140). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
