ATM (HGNC:795): (ATM serine/threonine kinase) The protein encoded by this gene belongs to the PI3/PI4-kinase family. This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 250742 control chromosomes. c.2113delT has been reported in the literature in individuals affected with Ataxia-Telangiectasia and has been subsequently cited by others (example, Gilad_1996a, Concannon_1997, Cavalieri_2008, Chessa_2009, Broccoletti_2011, Prodosmo_2013). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as pathogenic citing overlapping evidence utilized in the context of this evaluation. Based on the evidence outlined above, the variant was classified as pathogenic. -

The c.2113delT pathogenic mutation, located in coding exon 12 of the ATM gene, results from a deletion of one nucleotide at nucleotide position 2113, causing a translational frameshift with a predicted alternate stop codon (p.Y705Tfs*30). This alteration has been reported in a compound heterozygous state with other ATM alterations in Italian patients with classic ataxia-telangiectasia (A-T) (Gilad S et al. Hum. Mol. Genet. 1996 Apr;5:433-9; Chessa L et al. Orphanet J Rare Dis 2014 Jan;9:5; Broccoletti T et al. Eur. J. Neurol. 2011 Apr;18:564-70; Cavalieri S et al. Ann. Hum. Genet. 2008 Jan;72:10-8; Porcedda P et al. Cytometry A 2008 Jun;73:508-16). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
